Pre-School Registration

12:30pm - 3:30pm 
and
5:30pm - 7:00pm 

St. Stephen Martyr will hold registration for the 2008-2009 Pre-School Program in the DayCare facility of the SSAC Building.  Registration will be for children that will be four years old by October 1, 2008. 

Please bring the following to registration

1.  Copy of Birth Certificate
2.  Copy of Social Security Card
3.  Registration Form (Click Here to print in PDF)
4.  Child Profile (Click here to print in PDF)
5.  Copy of Current Health Insurance Card
6.  $40.00 registration fee
                (this fee will go towards purshasing supplies)
7.  Every Child must have Immunizations and 
     Eye Exam before School starts in August.   
     These Forms must be turned in by the first
     day of school.


The SSM Pre-School runs Monday through Thursday from 9:00 a.m. until 12:00 p.m.  Daycare is available before and after class as well as on Fridays.  Our annual schedule coincides with the St. Stephen Martyr School Schedule.

**The following guidelines will be used if more parents wish to enroll their child than can be accommodated.**

1. Those whose brothers or sisters currently attend St. Stephen Martyr.
2. Those children based on length of parish membership and stewardship participation.
3. Those whose brothers or sisters previously attended St. Stephen Martyr.
4. Members of parishes other than St. Stephen Martyr.
5. All others.
